Home Negative Pressure Wound Therapy (NPWT) for a Stage 4 Pressure Injury (Sacral Area)

During a prolonged hospital stay with extended bed rest, Grandma Chan developed a severe pressure injury over her sacral area. The hospital initiated wound debridement and Negative Pressure Wound Therapy (NPWT). However, when she was discharged—while the wound still required long-term repair—the family faced a major challenge.

They initially considered community nursing support, but learned that community services could only provide basic dressings and were unable to continue the more complex NPWT required for deep wounds. To avoid a prolonged and painful recovery, the family turned to YDCare for specialist home-based wound support.

Specialist Assessment & Home NPWT Support

After an in-home assessment, our wound care specialist nurse confirmed the wound was a Stage 4 pressure injury, with severe deep tissue damage and exposed bone. YDCare then implemented a structured home NPWT plan, including:

Equipment & materials: Arrangement of medical-grade NPWT device rental and dressings tailored to the wound condition.

Expert nursing care: Specialist nurses handled dressing changes and system checks at home (including seal integrity and pressure settings).

Family guidance: Clear teaching on basic device handling and what to monitor day-to-day.

24/7 support hotline: For urgent questions or unexpected issues—providing a crucial safety net. (In practice, because the plan was well set up early, the family rarely needed emergency calls.)

A 10-Month Healing Journey

This was a 10-month process requiring patience and consistent clinical oversight. For the first six months, NPWT supported the formation of healthy granulation tissue and new soft tissue, gradually covering the previously exposed bone. Over the final four months, the care plan transitioned to advanced wound dressings to protect new tissue and support final closure. Ultimately, the wound achieved stable healing, reducing the need for repeated hospital admissions.

For deep or extensive pressure injuries, basic dressings alone may not provide the optimal environment for repair. With professional assessment, combining NPWT (when clinically appropriate) and stage-specific advanced dressings can help establish a stable healing environment and support tissue regeneration.

If your loved one has a deep pressure sore that isn’t improving, contact YDCare to discuss whether NPWT may be suitable for home-based care.
