When Domestic Helpers Are Not Enough: Home Nursing Support for Post-Discharge Recovery

Mrs. Wong, an 80-year-old patient with liver cancer, was recently discharged after a one-month hospitalization at a private hospital. Although she lived with two domestic helpers who managed her activities of daily living (ADLs) and meals, her condition deteriorated significantly following her discharge.

Her mobility declined substantially, she became dependent on diapers, experienced constant fatigue, suffered from anorexia (poor appetite), and developed diaper rash (incontinence-associated dermatitis) along with peripheral edema (leg swelling). Comprehensive Nursing Assessment and Private Care

Mr. Wong reached out to YDCare for assistance. Upon learning about Mrs. Wong's condition, the Nurse Manager conducted a home visit to perform a comprehensive holistic assessment. This clinical evaluation covered:

  • - Vital signs (body temperature, blood pressure, and oxygen saturation)
  • - Skin integrity assessment

  • - Medication regimen management
  • - Home safety environment assessment

Following a detailed consultation with Mr. Wong and careful consideration of Mrs. Wong's condition and care needs, a 12-hour one-on-one private home care service provided by a Registered Nurse (RN) was arranged.

 

Clinical Interventions and Multi-Disciplinary Care

Once the service commenced, the rostered RN not only provided daily nursing care but also guided the domestic helpers on proper caregiving techniques to ensure that Mrs. Wong received consistent and appropriate support throughout the day.

To address the diaper rash, the RN applied prescribed ointments, monitored the skin condition daily, and provided regular clinical updates to the Nurse Manager. YDCare's Wound and Stoma Care Specialist Nurse also conducted regular follow-up assessments and provided specialist recommendations based on Mrs. Wong's skin condition.

To address her nutritional deficit, the RN actively encouraged Mrs. Wong to eat, utilizing a "small, frequent meals" approach to gradually improve her appetite and nutritional intake.

For the management of peripheral edema, the nurse performed regular gentle leg massages, assisted with leg elevation, and administered prescribed diuretics strictly according to the physician's instructions.

Within two weeks, the diaper rash showed significant improvement. Mrs. Wong's appetite recovered, her weight increased, and the leg swelling subsided considerably.

Mr. Wong expressed immense relief to the Nurse Manager, noting that the one-on-one professional home care service not only provided his wife with more stable and personalized care, but also substantially alleviated his caregiver stress.

 

Tailored Rehabilitation and Recovery

As Mrs. Wong's physical condition gradually stabilized, Mr. Wong hoped she could engage in more physical activity to accelerate her recovery.

The Nurse Manager subsequently referred a Physiotherapist to provide tailored home-based rehabilitation sessions. Collaborating with the Physiotherapist and Mr. Wong, the Nurse Manager co-created a structured daily schedule balancing rest and exercise, which was then implemented and supported by the home care RN.

Two weeks later, Mrs. Wong demonstrated significant improvement in her mobility and overall physical function.
