The value of home care is often reflected in the professionalism and attention to detail behind every service. At YDCare, every home visit is guided by a personalised care plan designed around the individual's health condition, daily routine and care needs.
While many older adults are still able to eat, walk and manage parts of their daily lives independently, this does not necessarily mean they no longer require support. What concerns many families is not whether their loved ones can take care of themselves, but whether someone is there to notice sudden health changes, provide timely assistance, and offer companionship when they are alone.
The following case of 80-year-old Uncle Wong (pseudonym) reflects the care needs faced by many families today.
At the age of 80, Uncle Wong remains mentally alert, mobile and largely independent in his daily life. However, beneath his seemingly stable lifestyle, he still requires ongoing health monitoring and emotional support, while his family continues to worry about his care arrangements.
Physically, Uncle Wong has hypertension and requires regular monitoring of his condition. In addition, due to his age and the potential effects of long-term medication, his daily walks outdoors also carry an increased risk of falls.
Emotionally, since the passing of his wife last year, Uncle Wong has spent most of his time at home with the support of a foreign domestic helper. While his daily living needs are well taken care of, language and cultural differences make meaningful communication difficult, leaving him with limited opportunities for companionship and conversation. Meanwhile, his son has a demanding work schedule and is only able to visit about once a week. For the family, the greatest concern has always been how to ensure Uncle Wong receives appropriate care, companionship and support when they cannot always be by his side.
After understanding Uncle Wong's health condition, daily routine and family situation, YDCare arranged a dedicated one-to-one Health Worker service, supported by a Nurse Manager who developed and continuously reviewed his personalised care plan. The goal was to help him maintain his familiar lifestyle while providing ongoing health monitoring, safety support and daily companionship.
As Uncle Wong has hypertension, the Health Worker receives a detailed case handover from the Nurse Manager before each visit to understand the day's care priorities and any special considerations. Upon arrival, blood pressure is measured and his physical condition is documented, allowing both the clinical team and his family to monitor his health and respond promptly if any concerns arise.
Uncle Wong enjoys taking daily walks in the nearby park and often returns to his company after lunch to handle personal matters. Rather than limiting his activities, the Health Worker accompanies him throughout these outings, paying close attention to his safety and physical condition while helping reduce the risk of falls. This enables him to continue his familiar daily routine without compromising his independence.
Attention is also given to the small details of his daily care. As Uncle Wong regularly takes traditional Chinese medicine, the Health Worker carefully prepares it at an appropriate temperature before serving it to help prevent accidental burns. During his afternoon rest, his condition is also closely observed to ensure his safety. These seemingly small details play an important role in reducing potential risks in everyday life.
Companionship is another important part of Uncle Wong's care. Following the loss of his wife, he had fewer opportunities for meaningful daily conversations. Through regular one-to-one visits, the Health Worker gradually built a trusting relationship with him and became someone he felt comfortable talking to. Beyond physical care, this ongoing companionship helped reduce feelings of loneliness and provided valuable emotional support.
With one-to-one home care, Uncle Wong has been able to maintain his usual lifestyle while benefiting from continuous health monitoring and enhanced safety support. His blood pressure is monitored regularly, and he receives professional accompaniment during daily walks, outings and rest periods, allowing him to continue living with greater confidence and peace of mind.
For his family, the ongoing support from the YDCare team has also eased much of the caregiving burden. Although they are unable to accompany him every day, they know he is receiving consistent care, health monitoring and companionship whenever needed.
This case demonstrates that one-to-one home care is about much more than assisting with daily living. For older adults who remain relatively independent but require ongoing health monitoring, safety support and companionship, personalised home care enables them to maintain their familiar lifestyle while giving families greater peace of mind.
